The contact form below is setup to send the email you put in it a confirmation email, as well as an email with the information you put in the form. This would normally send the visitor the thank you confirmation and it would send the owner of the website the information filled out in the form. It also requires the visitor to fill in the fields before it actually sends either of the emails.
